Fly Fishing Women of Minnesota (FFWMN)

Waiver and Release of Liability I fully understand and acknowledge that fly fishing, an outdoor activity, has inherent risks, dangers and hazards. My participation may involve casting a fly rod with a hook, standing along side a river or stream, standing and moving in a river or stream and crossing a river or stream and standing in a boat. Risks and dangers may arise from foreseeable and unforeseeable causes and such other risks, hazards and dangers that are integral to recreational activities that take place in a wilderness, water, outdoor or recreational environment. The following known risks are some of the dangers or potential for danger: drowning, moving water situations, falling in river or stream, deep mud along or in streams, lightning, falling objects, wild life, plants, insects, and distance to medical assistance. My participation in fly fishing activities may result in injury, illness, death or other ailments that could cause serious disability.


I, on behalf of myself, my personal representatives, my heirs, assigns, successors and all other persons who may claim through me or for me, release Fly Fishing Women of Minnesota and its representatives from any and all liability arising from accident, injury, illness or death or other consequences that I may suffer as a result of my voluntary participation in this activity


I have read the Waiver and Release and by signing it agree that it is my intent to exempt and release Fly Fishing Women of Minnesota and its representatives from liability from personal injury, property damage and wrongful death caused by ordinary negligence. It is specifically not my intent to release FFWMN from liability for Gross Negligence or for Intentional Torts

You hereby make and enter into each and every representation, waiver, release and indemnity described above on behalf of yourself, the minor, and any other parent or guardian of the minor. 2. Consenting to the use of your electronic signature in lieu of an original signature on paper. You have the right to request that you sign a paper copy instead. By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature. There is no penalty for withdrawing your consent. You should always make sure that we have a current email address in order to contact you regarding any changes, if necessary.
